Medical Release (any medical release is acceptable - does not need to be notarized)

Roster -- Please log back onto your GotSoccer account, created when you registered, and enter your players names, ID #s, Jersey Numbers and birthdates. IF YOU DO NOT COMPLETE YOUR ROSTER BEFORE REGISTRATION, you will have to sit at registration and fill out the roster before you leave!
